Requirements

  • Registered Nurse with current NMC registration (Mental Health, Learning Disability or Adult)
  • Completion of Mentorship Course or ENB equivalent
  • Proven experience working with complex health presentations
  • Skilled in clinical assessments, including risk evaluation
  • Experience supervising staff or students
  • Strong communication and teamwork abilities
  • Committed to compassionate, person-centred care

About the Role

As a Community Mental Health Nurse within the Specialist Older Adults Mental Health Service (SOAMHS) Triage Team, you’ll be the first point of clinical contact for referrals across West Sussex. You’ll assess, triage, and guide patients toward the right care pathways, ensuring they receive timely and appropriate support.

You’ll work remotely from home with regular in-person meetings at Drayton House and virtual team huddles through Teams. This setup combines independence with connection, letting you collaborate while maintaining flexibility.

Your insight will help shape safe, effective, and compassionate care for older adults navigating mental health challenges. It’s a role that demands both clinical expertise and genuine empathy—and rewards both.

  • Screen and triage referrals to identify the best clinical pathways for patients
  • Collaborate with families, carers, and referrers to develop person-centred care plans
