Requirements
- First Level Registered Nurse qualification: RNMH, RMN or RNLD
- Substantial experience at Band 5 or equivalent
- Experience supporting people over 65 with mental health needs
- Knowledge of the Mental Health Act and Mental Capacity Act
- Strong organisational and communication skills
- IT literacy and confidence with electronic care records
- A flexible approach and current driving licence
Salary Details
This role is offered at Band 6 Agenda for Change, with pay between £38,682 and £46,580 per annum (pro rata for part time).
About the Role
As a Community Mental Health Nurse in the Older Adults team, you’ll make a genuine difference in people’s lives. You’ll manage a caseload of patients with complex needs, supporting them to maintain independence and improve wellbeing.
This role gives you the freedom to work autonomously, yet you’ll always be part of a close-knit multidisciplinary team. You’ll collaborate with psychiatrists, occupational therapists and psychologists, making sure every decision is guided by evidence-based practice.

Key Responsibilities
- Assess and manage a caseload of community patients, improving quality of life
- Develop person-centred care plans that focus on recovery and wellbeing
- Deliver evidence-based clinical and psychological interventions
- Involve families and carers in planning to strengthen support networks
- Co-ordinate discharge planning, ensuring safe transitions of care
- Contribute to service development and champion high clinical standards
- Act as a mentor and role model for junior staff and colleagues
