Requirements
- RMN registration: Essential for practice in the UK
- Post-basic education: Up to postgraduate diploma level in relevant field
- Acute mental health experience: Experience working in adult mental health settings
- Assessment skills: Strong competence in Comprehensive Assessment, Risk Assessment, and Care & Treatment Planning
- Interpersonal skills: Able to communicate effectively with patients, families, and colleagues
- IT proficiency: Comfortable using electronic systems for patient care and reporting
- Welsh language: Desirable, but not essential; support available to meet requirements

Band 6: £39,263 – £47,280 per annum
About the Role
This is a chance to join a multi-disciplinary Community Mental Health Team in Newcastle Emlyn. You will coordinate care under the Mental Health Measure (Wales) 2010, supporting patients through recovery with a focus on person-centred outcomes. You’ll be actively involved in initial assessments, crisis intervention, and home treatment, ensuring each individual receives the most appropriate care. Travel between sites is required, so flexibility and reliability are key.

- Provide risk management and crisis intervention: Keep patients and communities safe
- Care coordination: Support seamless transitions across services for each patient
- Collaborate with multi-disciplinary teams: Ensure treatment plans are effective and holistic
- Promote recovery-focused outcomes: Help patients achieve meaningful improvements in wellbeing
- Maintain accurate records: Critical for patient safety and continuous service improvement
