About the Role
Hertfordshire Partnership University NHS Foundation Trust (HPFT) is recruiting a Community Outreach Support Worker to join our Specialist Community CAMHS team in South Hertfordshire. This role is ideal for an enthusiastic and compassionate individual with experience in mental health support for children and young people.
You will work closely with care coordinators, psychiatrists, and clinical nurse specialists, supporting young people with complex mental health needs in both clinic-based and community settings. Key Responsibilities
As a Community Outreach Support Worker, you will:
- Provide outreach support to young people, encouraging engagement with mental health services.
- Work closely with the CAMHS Nurse Specialist and multi-disciplinary team (MDT) to deliver high-quality care.
- Monitor and report changes in a young person’s mental health condition, ensuring accurate documentation.
- Offer practical and emotional support to young people, helping them with daily activities, school attendance, and appointments.
- Accompany clients to GP visits, outpatient clinics, and social care appointments as part of their care plan.
- Work flexibly in both clinic-based and community outreach settings, adapting to the needs of young people and their families.
- Ensure care services are delivered in a non-discriminatory and culturally sensitive manner.
- Participate in regular supervision and training to develop skills in child and adolescent mental health support.

What We’re Looking For
The ideal candidate will have:
- A good standard of education, including GCSEs in Maths and English (Grade C or above).
- Experience working with children and young people with mental health conditions in acute or community settings.
- The ability to assess young people and implement agreed care plans.
- Strong communication skills, both written and verbal, with the ability to engage young people and their families.
- The ability to work autonomously and manage distressing or challenging situations.
- A positive and compassionate approach to supporting young people’s mental health needs.
Desirable qualifications and experience:
- NVQ in Health and Social Care or equivalent qualification.
- Trainee Nursing Associate qualification.
- Basic knowledge of mental health laws and safeguarding frameworks.
