Job Summary
An exciting opportunity has arisen to join the Integrated Community Stroke Service as a Community Stroke Reviewing Officer. The role involves conducting six- and twelve-month post-stroke reviews, supporting rehabilitation, and working within a multidisciplinary team to provide high-quality care for stroke patients.
The award-winning Integrated Community Stroke Service operates from 8 AM – 5 PM, Monday to Friday, including Bank Holidays. Weekend working may be required in the future.
Key Responsibilities
- Conduct 6-month and 12-month post-stroke assessments to evaluate patient progress.
- Provide specialist assessments, support, and signposting to stroke survivors.
- Collaborate with Allied Health Professionals (AHPs) and other team members.
- Assess and support patients to improve independence following a stroke.
- Work with patients and families to develop care plans and rehabilitation strategies.
- Communicate effectively with a diverse patient group and their families.
- Maintain accurate documentation and records.
- Promote rehabilitation and independence through patient-centred care.
Requirements
Essential:
- NVQ Level 3 in Health or Health and Social Care or an equivalent qualification.
- Foundation degree or accredited CPD units in a relevant field.
- Experience in multi-disciplinary work (including voluntary work or related fields).
- Experience managing own caseload within a team setting.
- Experience working with stroke patients.
- Proficiency in record-keeping, report writing, and correspondence.
Desirable:
- Previous experience as an Assistant Practitioner or Rehab Support Worker.
- Experience in community stroke rehabilitation.
Knowledge & Skills
Essential:
- Good understanding of community care practices and stroke rehabilitation.
- Familiarity with health and social care services.
- Knowledge of single assessments and care planning.
- Ability to work collaboratively within a multi-disciplinary team.
- Awareness of rehabilitation techniques and assistive equipment.
- Strong verbal and written communication skills.
- Ability to work independently and manage caseload effectively.
Desirable:
- Experience using System One (electronic patient record system).
- Ability to empower patients and promote independence.
Additional Requirements
- Flexible working to meet service needs.
- Full UK driving licence and access to a car (essential).
- Ability to deal with challenging family situations.
