Job Overview
The Integrated Response Hub at NHFT is seeking a Community Mental Health Support Worker to join its team. This is an exciting opportunity to be part of a countywide mental health pathway that aims to improve access to services and provide essential support to individuals experiencing mental health crises. The post-holder will work with service users, supporting their recovery journey, achieving personal goals, and helping them reintegrate into the community.
The role involves working with mental health service users, creating and implementing support plans, and providing one-on-one interventions in a variety of community settings. The Band 4 worker will also be part of a team offering brief solution-focused therapy and crisis management.
Main Duties
- Liaise with mental health agencies (statutory and non-statutory) to identify individuals with mental health needs and coordinate their care.
- Support colleagues in risk and mental health assessments within the crisis care pathway.
- Provide practical support, safety planning, and crisis intervention to individuals and their carers.
- Assist in formulating and implementing care plans that promote independence and recovery for service users.
- Provide interventions in line with operational policies and improve the experience of patients using the crisis care pathway.
- Promote trauma-informed care and ensure service users are actively involved in the assessment and care planning process.
- Work collaboratively with acute hospitals, police, 3rd sector agencies, and other mental health services.
- Offer mental health awareness education to patients and carers, reducing stigma and improving understanding.
- Provide out-of-hours support as part of a countywide service, with shift work required across a 24/7 basis.
- Participate in the biopsychosocial assessment and co-production of care plans.

Person Specification
- Qualifications
- Essential: Evidence of competencies in Maths and English.
- Desirable: Experience in mental health, or further mental health training.
- Experience
- Essential: Experience in working with individuals with mental health issues or complex needs.
- Desirable: Experience in telephone communication and in varied settings.
