Requirements
- NVQ Level 3 in Care or equivalent experience and skills
- Previous experience in a mental health or social care setting
- Ability to work well as part of a team

Salary Details
The role is paid at Band 4 under the NHS Agenda for Change pay scheme, ranging from 27,485 to 30,162 per year.
About the Role
As a community support worker, you’ll join the Intensive Support Team to work directly with adults and older people, including those living with dementia. Your role is about keeping people safe in their own homes, reducing admissions, and helping smooth transitions after discharge. Shifts will run weekdays from 08:30 to 19:00 and weekends from 09:00 to 17:00, ensuring round-the-clock care when it’s most needed. Every interaction matters here, as you’ll be the familiar face making sure care plans work in practice and that service users feel supported every step of the way.
Key Responsibilities
- Carry out care plans and ensure service users receive the right level of support in the community
- Monitor the quality of care and check in on satisfaction levels to keep standards high
- Liaise with inpatient teams to ease the transition from hospital to home
- Keep accurate and up-to-date records that reflect each individual’s journey
- Take part in regular supervision to strengthen skills and stay aligned with best practice
