Requirements
- GCSEs in English and Maths at minimum Grade 4 or equivalent
- NVQ or similar training within an acute hospital setting
- Previous ward-based discharge planning experience
- Solid understanding of health care systems and multi-agency discharge planning
- Intermediate IT skills including use of Outlook, Word, Excel
- Valid driver’s license with access to a vehicle
- Effective communication skills both verbal and written
- Comfortable managing competing priorities without losing focus or compassion
Salary Details
The salary range for this full-time role is 26,530 to 29,114 per year, based on experience and responsibility within the team.
About the Role
This isn’t your average office job. As a Complex Case Support Worker, you’ll be the boots-on-the-ground presence driving efficient patient discharges at East Lancashire Hospitals NHS Trust. Your role is key in reducing hospital stays and coordinating safe, timely transitions out of care. Working across Burnley General Hospital and other ELHT sites, you’ll take charge of a varied caseload, engage directly with patients and multidisciplinary teams, and help break down barriers to discharge. Every action you take will help a patient get home sooner and safer.
From chasing referrals to inputting live data, from supporting ward staff to identifying risks early, this job demands attention to detail, empathy, and speed. And you won’t do it alone — you’ll report back to Complex Case Managers who trust you to handle the day-to-day while flagging when things go sideways. It’s a people-first role in a fast-paced system. Key Responsibilities
- Support patients from admission to discharge with responsive case management
- Work independently across multiple ELHT sites to manage discharge plans
- Advise ward staff and escalate complex issues to senior managers
- Coordinate with ward teams, patients, social care, and other services
- Maintain accurate patient records and referral data using hospital systems
- Respond quickly to calls and discharge-related alerts
- Input trusted assessment documents and flag risks as needed
- Monitor and report on discharge delays to keep processes moving
- Cover colleague absences and assist with team-wide improvements
