Requirements
- GCSE level 4 or equivalent in English and Maths
- NVQ or equivalent training in an acute care setting
- Experience in ward-based discharge planning
- Health care background with solid MDT collaboration
- Great communicator with confident writing and interpersonal skills
- Intermediate IT skills – including Outlook, Word, Excel
- Able to travel between sites with access to a vehicle
- Self-motivated and organised – able to manage competing demands calmly
Salary Details
This role offers a banded NHS salary from 27,485 to 30,162 per year, based on experience and qualifications, plus access to additional benefits.
About the Role
You’ll work at the centre of patient care transitions, supporting the Integrated Discharge Service to ensure timely, safe hospital discharges. Based at Burnley General Hospital but traveling across sites, you’ll bring heart and logic to each case. From managing dynamic discharge plans to liaising with social services, you’ll play a key role in delivering impactful work that gets patients home safely and efficiently.
You won’t just be pushing paper. You’ll work directly with patients, families, nurses and community partners to untangle complex cases and keep things moving. This is your chance to grow a meaningful career while making sure every patient gets the right care at the right time.
Key Responsibilities
- Help manage complex discharge cases across multiple hospital wards
- Work closely with social workers, ward managers and healthcare teams
- Escalate delays and disputes early to resolve issues quickly
- Maintain accurate patient records using NHS and social services systems
- Spot risks in referrals and act fast to keep discharges on track
- Offer trusted advice to ward staff and update MDTs regularly
- Keep services flowing by managing caseloads independently
- Contribute to data reporting, reviews and improvement projects
