Requirements

  • Educated to Diploma level or equivalent through training and work experience
  • NVQ Level 3 in Health or Social Care
  • Ability to stay calm under pressure and juggle multiple priorities
  • Skilled at using Word, Excel, Outlook, and managing admin tasks
  • Great communication skills, verbal and written
  • Able to handle difficult situations with empathy and professionalism
  • Flexible for shift-based work, including weekends

Salary Details

The post is offered on NHS Band 4, starting from 26,530 up to 29,114 per year depending on experience.

About the Role

This isn’t a background role. As a Discharge Facilitator, you’re the person making sure no patient is left behind when they’re medically fit to go. You’ll be right at the centre of patient flow coordination, managing the logistics that get people discharged safely and on time.

From liaising with families and community services to coordinating transport and assessing aftercare needs, your work directly affects the patient experience. In short, you’ll be keeping things moving—every single day.

Key Responsibilities

  • Coordinate the safe and timely discharge of patients ready to leave hospital
  • Work closely with assessment teams and wards to remove bottlenecks
  • Speak with patients and families to arrange aftercare needs and support
  • Problem-solve in real time to keep things flowing smoothly across the hospital
  • Liaise with external services, transport teams, and community providers
  • Participate in weekend shifts and be part of a 7-day rota team
  • Handle and prioritise fast-changing patient flow demands effectively
