Requirements
- Registered Nurse (First level) with current NMC registration
- Degree-level qualification and a recognised teaching qualification
- Excellent communication and time management skills
- Experience in discharge coordination or elderly care nursing
- Ability to motivate others and work collaboratively with multidisciplinary teams
- Understanding of continuing health and social care processes
- Car driver with the ability to travel between sites when needed
- Commitment to continued professional development
Salary Details
This role sits at Band 6, offering £38,682 – £46,580 per annum pro rata, with access to NHS pension and benefits.
About the Role
This is a specialist nursing position designed to ensure patients are discharged safely, efficiently, and with dignity. You’ll coordinate discharge plans, assess patient readiness, and work closely with ward teams, the discharge hub, and community services. Your input will reduce hospital stays, improve outcomes, and strengthen patient experiences across the Trust.
Each day, you’ll contribute to the bigger picture—helping patients return home with the right support in place. You’ll attend multidisciplinary meetings, advise staff on complex cases, and lead on implementing discharge policies that make a tangible difference.
Key Responsibilities
- Lead and coordinate discharge planning for patients with complex needs to promote timely transitions home
- Provide specialist nursing advice and training to ward teams and other health professionals
- Conduct Reason to Reside checks and ensure accurate patient documentation
- Participate in Length of Stay (LOS) reviews and meetings to support efficiency
- Collaborate with community teams to build sustainable post-discharge care pathways
- Contribute to service improvements and audit initiatives that enhance patient experience
