About the Role
We are seeking to recruit individuals with a passion for helping young people and their families to get the right support from Health, Social Care, Education, and other services. You will join an established team of Dynamic Support Keyworkers in Liverpool, working across a wide range of settings to coordinate and provide challenge to the system, ensuring that young people and their families can access the right support to achieve better outcomes and remain safe in the community.
Key Responsibilities
- Be a point of contact for the family and involved professionals
- Have regular contact with the family, listen to, support, and work in partnership with them
- Provide information and advice, helping families build confidence and control
- Work closely with families and agencies to prevent admission to Mental Health Settings
- Support the Care, Education, and Treatment Review (CETR) process
- Develop relationships with internal and external stakeholders
- Ensure compliance with relevant legislation
