Job Description
The Glossop Health Team, part of the Glossop Primary Care Network, is seeking an experienced Health Care Assistant to join their multidisciplinary team. This role involves managing patients with acute and chronic conditions, providing treatment, preventative care, health promotion, and patient education both in the surgery and through home visits. The Health Care Assistant will perform baseline observations such as pulse oximetry, blood pressure, phlebotomy, BMI, and temperature measurements, recording findings accurately. They will also assist in managing chronic diseases and delivering opportunistic health promotion.
This position is initially offered on a 12-month contractual basis, with the possibility of extension. A travel allowance will be provided for home visits.

Glossop Health Team supports five practices in the Glossop area, catering to approximately 33,350 patients. The team includes social prescribing link workers, mental health workers, paramedics, pharmacists, and other roles. This collaborative environment aims to deliver coordinated care to the patient community.
Key Responsibilities
- Manage specific cohorts of patients with acute and chronic conditions
- Deliver care within scope of practice in the surgery and during home visits
- Assist clinical staff in providing treatment, preventative care, health promotion, and patient education
- Conduct baseline observations and record findings accurately
- Support management of chronic diseases

Person Specification
Experience
Essential:
- Good communication skills (written & verbal)
- Good organisational skills
- Good customer service skills
- Ability to work flexibly to meet service demands
Desirable:
- Interest or previous experience in caring for patients in an acute setting
- Basic knowledge of IT packages
- Experience working in a team
Qualifications
Essential:
- Level 1 literacy and numeracy (GCSE English and Maths Grade D-G)
Desirable:
- NVQ level 3 in Healthcare
