Requirements
- Level 3 HCSW Apprenticeship in Primary Care
- Strong verbal and written communication skills
- Proven ability to support long term condition management
- Experience in community or primary care settings
- Comfortable with phlebotomy, ECGs, wound care, and health checks
- Confidence in working independently and collaboratively
- Open to working across two nearby practice sites
- Desirable but not essential: Immunisation training, specialist clinical skills, clinical supervision experience

About the Role
This is more than a tick-box healthcare assistant job. As part of a dedicated nursing team, you’ll be at the heart of patient care. One day you might be doing diabetic foot checks, the next assisting in minor ops or supporting long-term condition reviews. You’ll help people stay healthier and feel heard. You’ll play a big role in clinical continuity, ensuring the practice runs smoothly and patients get timely, compassionate support. Key Responsibilities
- Carry out vital sign checks, ECGs, phlebotomy, and wound care to inform patient treatment
- Assist with long-term condition reviews and health promotion activities
- Support emergency response situations and minor surgery procedures
- Maintain clear and detailed records that support excellent continuity of care
- Liaise closely with GPs, nurses, and pharmacists to tailor care for each patient
- Supervise or support learners and phlebotomists when needed
- Follow all policies and contribute to clinical audits and service improvement
- Help ensure efficient use of time and resources without compromising care
Disclosure and Barring Service Check
This role requires a DBS check as part of the Rehabilitation of Offenders Act (Exceptions Order) 1975 to ensure patient and staff safety.
