About the Role
The Health Care Support Worker Band 2 position in the Complex Care at Home Team involves providing essential care to patients with complex needs in their own homes. Under the guidance of registered nurses, the worker assists in delivering high-quality, compassionate care within a flexible service environment. This position plays a key role in promoting patient-centered care in line with the Hospital-2-Home initiative, ensuring that individuals receive dedicated support tailored to their healthcare needs.

Key Responsibilities

  • Support registered nurses by delivering patient care as outlined in care plans
  • Provide compassionate assistance to individuals with complex health needs in their own homes
  • Maintain strong communication and flexibility within a dedicated care team
  • Commit to ongoing personal development and training
  • Ensure efficient travel to patient sites within Aneurin Bevan Health Board’s coverage area
