Requirements
- NVQ Level 3 or equivalent experience
- Trained or willing to train in phlebotomy and venepuncture
- Familiarity with mental health and physical wellbeing issues
- Experience in healthcare settings, especially in the community
- Excellent written, verbal and non-verbal communication skills
- Basic IT and standard keyboard skills
- A full UK driving licence and a vehicle for business use (reasonable adjustments apply)
Salary Details
This position offers a competitive NHS Band 4 salary between 26,530 and 29,114 depending on experience, paid monthly.
About the Role
Here’s your chance to tackle something real. People living with serious mental illness often miss out on essential physical health care, dying years earlier than the general population. This impactful work helps close that gap.
As a Health Improvement Worker, you’ll work across the Gedling area, identifying and managing cardio-metabolic risks in patients with serious mental illness. Working shoulder to shoulder with GPs and mental health professionals, you’ll coordinate care, build trust, and encourage recovery through real action.
You won’t just be ticking boxes. You’ll be enabling people to take back control of their lives. And you’ll be supported every step of the way.
Key Responsibilities
- Carry out targeted health interventions in the community
- Work collaboratively with secondary mental health care and primary care teams
- Help service users understand and manage physical health risks
- Use observation skills to detect health changes early
- Build rapport and trust with patients through empathy and patience
- Coordinate support plans to improve both physical and mental wellbeing
- Act as a bridge between services, keeping communication clear and active
Disclosure and Barring Service Check
All successful applicants must complete a Disclosure and Barring Service (DBS) check. A fee of 26.40 (standard) or 54.40 (enhanced) will be deducted from salary over the first two months.
