Aintree Core 24 operates as part of the 24/7 Urgent Care Model within Mersey Care NHS Foundation Trust, delivering mental health assessment, intensive community interventions, and guidance. These services are provided at Southport District General Hospital, Aintree Hospital, The Royal at Liverpool, Whiston Hospital, and Warrington Hospital.

Main Duties of the Job
The role involves carrying out assigned duties under the guidance and indirect supervision of qualified staff, providing direct care and observations of service users. Reporting observations regarding mental state and risk factors to qualified staff is essential. The role includes working with service users individually and in groups as outlined in their care plans and under the direction of the lead professional.

Detailed Job Description and Main Responsibilities
The Hospital Mental Health Liaison Team practitioners collaborate with multidisciplinary teams in acute hospital settings, including medical and nursing teams, Community Mental Health Teams, Local Authority services, and the Voluntary Sector. The role supports patients presenting with mental health conditions and potential physical comorbidities, often in crisis.

Responsibilities include conducting comprehensive mental health assessments, contributing to discharge planning, and fostering community links across health and social care. Practitioners are expected to work independently, making informed clinical decisions regarding signposting and discharge processes. Supportive observations and assistance with the 136 process in A&E departments are integral to the role.

Qualifications

  • Desirable: NVQ Level 2 or willingness to work towards
  • Good standard of education

Experience

  • Desirable: Experience in a care setting or with individuals experiencing mental health challenges

Knowledge and Skills

  • Essential: Effective communication, confidentiality understanding, teamwork, non-judgmental attitude, initiative under supervision, and adaptability.
  • Desirable: Knowledge of breakaway techniques and de-escalation strategies.
