Key Responsibilities

• Provide person-centred care that respects personal routines and preferences
• Visit clients’ homes to offer physical, emotional, and practical support
• Assist with bathing, dressing, toileting, grooming, catheter and stoma care
• Administer prescribed medication safely and record accurately
• Use hoists and mobility aids with confidence and care
• Monitor health changes and report concerns to the care coordinator
• Offer companionship and create safe, trusting relationships
• Keep care plans updated and follow guidelines precisely
