Working within the Swanshurst IMT, you will provide essential nursing care to patients in their own homes. Responsibilities encompass delivering personalized clinical care, conducting holistic assessments, and ensuring patients’ independence and well-being. Collaboration with a multidisciplinary team is vital for providing outstanding care. - Key Responsibilities:
- Administer clinical care directly to patients in their homes.
- Conduct thorough and holistic assessments for personalized care delivery.
- Work effectively with multidisciplinary team members to meet patient needs.
- Adhere to an 8am–6pm shift pattern, transitioning to 8am–8pm within a year.
- Weekend and bank holiday shifts are part of the role.
