Salary Details

This Band 6 role offers £44,485 to £52,521 per year including high cost allowance paid yearly.

About the Role

This role supports patients experiencing homelessness who arrive at the Princess Royal University Hospital. The work blends advocacy, practical support and collaborative planning. It centres on safer hospital discharge and improved health outcomes. The team links people to primary care, mental health care and addiction support. They help patients understand their choices and build connections that reduce readmissions. The pace is steady and hands on. The conversations are honest. The impact is immediate.

You’ll work closely with hospital teams and community partners so each patient leaves with a plan that feels realistic and safe. You’ll guide colleagues on issues linked to homelessness. Key Responsibilities

• Support holistic assessments to understand health, housing and social needs so each discharge plan feels safe
• Build relationships with local authorities and wider agencies so patients receive smoother support after leaving hospital
• Provide guidance to clinicians so decisions around homelessness are well informed
• Complete Duty to Refer tasks so patients are linked with housing teams when needed
• Work with Transfer of Care teams so communication stays timely and clear
• Offer education to colleagues so the hospital’s approach to homelessness keeps improving
