This role supports patients experiencing homelessness who arrive at the Princess Royal University Hospital. The work blends advocacy, practical support and collaborative planning. It centres on safer hospital discharge and improved health outcomes. The team links people to primary care, mental health care and addiction support. You’ll work closely with hospital teams and community partners so each patient leaves with a plan that feels realistic and safe. You’ll guide colleagues on issues linked to homelessness. Key Responsibilities

  • Support holistic assessments to understand health, housing and social needs so each discharge plan feels safe
  • Build relationships with local authorities and wider agencies so patients receive smoother support after leaving hospital

  • Complete Duty to Refer tasks so patients are linked with housing teams when needed
  • Work with Transfer of Care teams so communication stays timely and clear
  • Offer education to colleagues so the hospital’s approach to homelessness keeps improving
