Requirements
- Registered RGN, RMN, HCPC therapist, or Social Worker
- Evidence of recent professional development
- Experience in community or hospital care with holistic patient approaches
- Confidence in case management models and person centred planning
- Knowledge of community care legislation and health policy
- Ability to lead and collaborate across multi agency teams
- Car driver with access to a vehicle

This post is offered on Agenda for Change Band 7, with a salary of £47,810 to £54,710 per year, depending on experience.
About the Role
This isn’t just a case manager job, it’s a chance to be a lifeline. Working as part of the Integrated Neighbourhood Team, you’ll support people experiencing homelessness or at risk of it, many with complex health and social needs. Your work will focus on building trust, coordinating multi agency support, and ensuring no one falls through the cracks.
Each day you’ll bring professionals together, from GPs to housing teams, to create solutions that help people find stability and health. You’ll promote holistic care, reduce hospital admissions, and ultimately change outcomes for people who are often left behind. Key Responsibilities
- Coordinate care plans that address health and housing needs, helping people stay well in the community
- Lead and chair multidisciplinary meetings where real solutions are agreed and acted on
- Work with GPs, social care and housing partners to deliver joined up services that matter
- Support early discharge from hospital so patients return to safe environments quicker
- Promote self care and independence to reduce reliance on hospital based care
- Act as a point of contact for staff and patients, ensuring consistent communication and continuity
