Requirements

  • Degree in Social Work, registered with Social Work England
  • Solid experience supporting older adults with mental health needs
  • Good grasp of the Care Act, Mental Health Act, and Mental Capacity Act/DOLS
  • Comfortable leading discharge planning from both community and hospital settings
  • Confident writing reports and supporting adult safeguarding processes
  • Natural team player with a drive for impactful work

Salary Details

£42,939 – £50,697 annually, depending on experience. You’ll also enjoy NHS benefits and access to future progression routes.

About the Role

This isn’t your average discharge role. As a Hospital Discharge Mental Health Social Worker, you’ll play a key part in smoothing the discharge flow for older adults in City and Hackney. You’ll get stuck into ward and community-based work, shaping recovery journeys alongside a brilliant multidisciplinary team.
From tackling social barriers to guiding patients through complex mental health transitions, your voice as a social worker matters here. You’ll help deliver a discharge-to-assess model, contribute to better patient outcomes, and keep the social perspective front and centre. If you’re someone who gets fired up about doing the work that actually changes lives – this might just be it.

Key Responsibilities

  • Act as the key discharge social work lead across inpatient and general hospital wards
  • Represent the social model in all interventions and multidisciplinary planning
  • Work closely with patients and carers to identify strengths and promote independence
  • Support adult safeguarding cases with confidence and professionalism
  • Handle follow-up reviews post discharge before transitioning cases to the wider community team
  • Actively reduce delays and improve the discharge process from a whole-system perspective
