Requirements

  • Level 5 qualification – Foundation Degree or similar in Health and Social Care
  • Experience in a relevant care or health-based role
  • Familiarity with HCPC code of conduct
  • Strong teamwork and communication skills
  • Proactive, can-do attitude when working across agencies

Salary Details

The salary range is £29,970 to £36,483 per year, with NHS benefits and banding based on experience and role fit.

About the Role

They need someone who can coordinate, deliver and review therapeutic stroke support in both hospital and community spaces. You’ll join a multi-professional team making sure patients get rehab and care that fits around their lives – faster, better, closer to home. Key Responsibilities

  • Provide flexible therapy services across acute and community stroke pathways
  • Coordinate personalised care in partnership with families and agencies
  • Contribute to service evaluation and quality improvement projects

  • Advocate for patient dignity, comfort, and control during recovery
