Requirements
- NVQ Level 3 or equivalent in health or social care, or a foundation degree with a plan to complete within 2 years
- Hands-on experience in mental health or social care, especially in a community setting
- Strong knowledge of the Mental Health Act, Homelessness Reduction Act, and safeguarding protocols
- Familiarity with recovery-based approaches and personalised care planning
- Comfortable using IT tools for care documentation and communication
- Confident in partnership work with third-sector groups and public services
- Clear understanding of social inclusion and wellbeing frameworks

About the Role
This Mental Health Link Worker role is more than just a job—it’s a chance to directly support people living with Serious Mental Illness (SMI) by helping them build stronger connections and reclaim their place in the community. Based in Stroud, with some travel across Gloucestershire, you’ll be at the heart of shaping meaningful outcomes through social inclusion and collaborative care.
Working within Community Mental Health Teams (CMHT), the role helps break down the barriers people with mental health conditions face. Whether it’s physical inactivity, housing struggles, or loneliness, you’ll work side by side with individuals to co-create personal goals that reflect their values, not just their diagnoses. This job suits someone who’s action-oriented, community-minded, and driven to make change where it matters most.
Key Responsibilities
- Build relationships with VCSE partners to support emotional wellbeing and early intervention
- Boost confidence in community groups to identify and manage emotional distress
- Improve referral pathways between third sector services and NHS mental health teams
- Carry a light caseload to directly support navigation and access across services
- Lead personalisation efforts, ensuring care aligns with each person’s goals and motivations
- Participate in Locality Community Partnership meetings to co-design care plans
- Act as a bridge between GP practices and mental health services, strengthening integrated care
