Job Summary

Cheshire and Wirral Partnership NHS Foundation Trust (CWP) is offering an exciting opportunity for a full-time Band 6 Mental Health Practitioner to join the liaison psychiatry team based in Chester. The role extends across various services, including Urgent Care Mental Health First Response Services, Crisis Resolution Home Treatment, and Liaison & Diversion Services. You will be working in dynamic environments, offering comprehensive biopsychosocial assessments and interventions for individuals presenting with co-morbid psychiatric conditions.

Main Duties of the Job

  • Work as part of a multidisciplinary liaison team to provide biopsychosocial assessments and interventions in acute hospital settings.
  • Provide comprehensive mental health assessments for individuals experiencing crises in community settings, collaborating with police, NWAS, and Local Authorities.
  • Deliver consultations and psychosocial interventions to patients aged 16 and older across various healthcare settings.
  • Collaborate with teams such as Crisis Triage & Response, Liaison Psychiatry, and Street Triage Services.

Key Responsibilities

Requirements
