Requirements
- A professional qualification in Adult Mental Health such as RMN, Occupational Therapy, Social Work or Therapy
- Current UK registration with the relevant professional body
- Solid post-qualification experience supporting adults with mental health difficulties
- Confidence in assessment, planning and co-ordination of care
- Strong communication and teamwork skills to support GPs and patients alike
- Ability to travel independently across a large geographical area
- A genuine passion for community mental health services
Salary Details
This is a Band 6 Agenda for Change role, with pay ranging from £38,682 to £46,580 per year, pro rata.
About the Role
As a Mental Health Practitioner, you’ll be embedded within a primary care team, working closely with GPs, carers and patients. You’ll carry out mental health assessments, provide brief therapeutic interventions, and act as the bridge between primary care and wider community services. The real heart of the role is impact: making sure that patients don’t slip through the cracks, and that support is joined up, fast, and meaningful.

Key Responsibilities
- Assess mental health needs in primary care to identify the right pathway quickly
- Deliver short-term interventions to support people with common mental health conditions
- Promote mental health awareness across local communities and GP practices
- Advise and support GPs so patients access the right care faster
- Strengthen links between services to reduce waiting times and improve continuity of care
- Contribute to care planning and risk management that prioritises safety and recovery
