Requirements
- Recognised Social Work Qualification – MSW, DSW, or CQSW
- Active Registration with GSCC
- Mental Health Experience – Substantial work with adult mental health services
- Risk Assessment Skills – Able to carry out detailed and thoughtful evaluations
- Discharge Planning Expertise – Confident in leading smooth transitions in care
- Safeguarding Knowledge – Solid grasp of safeguarding for adults and children
- Team Player – Proven record in a multi-agency setup, co-working with GPs and third-sector partners

Your days will revolve around co-designing safety and care plans with adults navigating severe and enduring mental health challenges. Key Responsibilities
- Co-create personalised care plans with service users and carers that reflect real goals
- Work closely with GPs, statutory services, and third-sector teams to ensure coordinated care
- Identify and manage risks related to abuse, self-harm, or neglect with clarity and compassion
- Lead discharge planning to support safe and timely transitions back into community living
- Champion safeguarding as a daily practice, not just a compliance task
- Contribute to a lively, collaborative team culture where professional learning is valued
