Requirements
- Previous experience in health or social care at Band 3 or equivalent
- Strong interpersonal and communication skills with a caring, sensitive attitude
- Proven ability to plan, prioritise, and work autonomously
- Awareness of referral pathways and patient support systems
- Confident with data collection and coordination across teams
- GCSEs in Maths and English (Grade 4/C or above) or equivalent
- NVQ/QCF Level 3 in Health or Social Care or working towards it
- Solid IT literacy, especially in Word, Excel, and data governance
- Coaching, teaching, or counselling exposure desirable
Salary Details
This is a Band 3 equivalent role with pay aligned to NHS Agenda for Change. Final salary depends on experience and qualifications.
About the Role
So here’s the deal. If you’re looking to do work that actually matters—this is it. As a Metastatic Breast Cancer Care Coordinator, you’ll be a constant point of support for patients and families navigating one of the toughest moments of their lives.
You won’t just assist with clinical tasks—you’ll connect, reassure, coordinate, and advocate. Every call you make, every referral you manage, every care plan you coordinate… it all adds up to a real difference. Working closely with Weston Park Hospital, you’ll collaborate with Clinical Nurse Specialists and wider teams, ensuring no one falls through the cracks.
This isn’t a job where you watch from the sidelines. You’ll be part of the frontline of compassionate, human-centred cancer care—making sure patients feel seen, supported, and safe.
Key Responsibilities
- Provide consistent support for patients across their cancer care pathway
- Coordinate care and signpost patients to specialist resources
- Maintain open communication with families, carers, and healthcare professionals
- Support the Clinical Nurse Specialists in daily patient management
- Liaise across services to enable seamless, holistic care
- Help collect and analyse patient data to improve outcomes
- Prioritise and manage your caseload independently, while knowing when to seek help
