Job Overview
This opportunity seeks a highly motivated, experienced, and enthusiastic Registered Nurse with expertise in compassionate end-of-life care. The role involves assessing and evaluating patient conditions in accordance with the National Framework for NHS Continuing Healthcare (CHC) Fast Track Pathway.
The role includes arranging and closely monitoring personalized care, working with individuals and their families during challenging times, and delivering high standards of care as part of a collaborative team. Regular face-to-face visits and completion of Decision Support Tools (DSTs) are expected.
Main Duties
- Working autonomously within the Continuing Healthcare Fast Track team and managing personal workload based on patient needs and service requirements.
- Receiving, triaging, and accepting referrals for patients with rapidly deteriorating conditions entering the terminal phase.
- Managing a personal caseload, commissioning care packages in line with policies, and supporting patients and families with high-quality, compassionate care.
- Acting as a specialist resource for colleagues and multi-disciplinary teams, providing advice on Fast Track completion, supporting education, clinical work, audits, and service development.
- Communicating sensitive information effectively with patients, providers, and carers using diverse communication methods to ensure understanding.
Working Environment

- First Level Registration with NMC.
