Requirements
- NVQ Level 2 or 3 in Health or Social Care, or equivalent
- Background in Health and Social Care – personal or professional
- Experience as a Therapy Technical Instructor or Assistant Practitioner
- Able to assess and treat non-complex patients independently
- Good communicator and team player – flexible and adaptable in busy environments
- Manual handling skills – with appropriate training
- Comfortable working unsupervised
- Nice to Have: Care Certificate, mentoring experience, basic IT skills, or voluntary care work
Salary Details
£26,530 to £29,114 a year depending on experience and post location. Paid monthly. Includes full NHS benefits.
About the Role
This is one of those impactful jobs where what you do really matters. Whether you’re based at OPAL House or rotate between the acute frailty unit, ED, and the Clinical Decisions Unit, you’ll play a key role in supporting older people as they recover from acute illness.
This isn’t just about rehab – it’s about restoration. After specialist training, you’ll help patients get back on their feet, teaching practical skills, carrying out assessments, and tailoring treatment plans that enable safe discharge home. You’ll be the steady hand in a moment of vulnerability, giving people the confidence to live independently again.
You’ll collaborate with Occupational Therapists, Geriatricians, frailty nurses, and the integrated discharge team. There’s variety here, a team that’s got your back, and room to grow into a more autonomous role over time.

Key Responsibilities
- Assess and treat non-complex patients under guidance from qualified clinicians
- Deliver therapeutic group and 1:1 activities using both creative and technical skills
- Modify treatment programmes to fit patient progress
- Coordinate discharge planning and ensure follow-up services are arranged
- Liaise with a wide multidisciplinary team to streamline the patient journey
- Support documentation and care plans throughout the discharge pathway
Disclosure and Barring Service Check
This role requires a DBS check under the Rehabilitation of Offenders Act (Exceptions Order) 1975. Candidates will need to provide disclosure of any previous criminal convictions.
