Requirements

  • NMC registration with a degree in Adult Nursing or as a Nursing Associate
  • Experience working in health and social care, preferably mental health
  • Strong written and verbal communication
  • Confident assessing patient needs and using clinical judgement
  • Tech-savvy with the ability to input and manage data digitally

Salary Details

This role is part of the NHS Band 4–5 scale, offering a salary between £31,081 and £33,665, depending on experience.

About the Role

This is more than a clinical post. It’s a mission to level the playing field for patients with severe mental illness who often face worse physical health outcomes. You’ll be the point of contact for physical health within the community mental health team, carrying out structured assessments, screenings, and follow-ups as part of each patient’s personalised care plan.

Working under the guidance of the Physical Health Clinical Nurse Specialist, you’ll support everything from routine health checks to advising on lifestyle improvements. And you won’t do it alone. You’ll collaborate with multidisciplinary teams, advocate for patients during GP referrals, and play a part in shaping a more integrated care system.

Key Responsibilities

  • Carry out specialist physical health checks and screenings using NHS tools
  • Collaborate with mental health teams to ensure holistic care plans
  • Monitor ongoing conditions and guide patients to the right services
  • Record clear, timely notes in the Trust’s electronic system (Rio)
  • Support patients in making healthy lifestyle choices that stick
  • Promote physical health education within service user groups
  • Champion non-discrimination in all aspects of care delivery
