Requirements
- Ability to proactively and independently manage a caseload
- Completion of the Care Certificate or equivalent
- NVQ Level 3 in Mental Health or equivalent experience
- Excellent written and oral communication skills
- Good IT skills

About the Role
The Primary Care Worker will work to build trusting and collaborative relationships with service users and their carers. They will assist in the formulation, implementation, and monitoring of recovery care plans, attend and contribute to multi-disciplinary meetings, and provide low-intensity psychological interventions to support service users in achieving personal goals.
Key Responsibilities
- Build trusting relationships with service users and carers
- Assist in the formulation and monitoring of recovery care plans
- Provide low-intensity psychological interventions
- Act as a link between General Practitioners, housing, carers centre, and other agencies
- Support the recovery journey of those being discharged to primary care
