About the Role:
As a Healthcare Support Worker with the Complex Care at Home Team, you’ll provide vital care to patients with complex health needs in the comfort of their own homes. You’ll work under the guidance of registered nurses, delivering personalized care and support. This role offers the chance to develop your healthcare skills and contribute positively to the well-being of individuals in the community. You will be part of a growing and successful team, working evening shifts that provide consistency and predictability in your schedule.
Key Responsibilities:
- Providing personal care and support to patients in their homes, empowering them to maintain their independence.
- Working under the supervision of registered nurses, following care plans and ensuring patient safety.
- Assisting with medication administration, monitoring vital signs, and other essential healthcare tasks. Your diligence will ensure patients receive the necessary care and attention.
- Communicating effectively with patients, families, and other healthcare professionals.
- Contributing to a positive and supportive team environment, fostering collaboration and shared learning.
- Driving patient vehicles to facilitate their care and transport needs.
- Maintaining accurate records and documentation.
