About the Role
Wigton Hospital is seeking a dedicated Rehabilitation Assistant to join the Intermediate Care Service (ICS) team, delivering top-tier rehabilitation and therapy-focused care. You will support patients following stays in acute and community hospitals, helping them regain independence and preventing inappropriate hospital admissions.

You will work to facilitate smooth transfers from hospitals to interim 24-hour bedded settings, while promoting safe and supported discharges. Your duties will include multidisciplinary assessment and treatment, offering advice and referrals to ensure patients receive the appropriate level of support.

As part of this role, you will also be working with the ICS team to prevent hospital admissions by providing high-quality care in community settings.

Key Responsibilities

  • Work as a lone worker without direct supervision, under the guidance of qualified staff
  • Assist in the assessment, implementation, and evaluation of care plans
  • Carry out simple assessments and prescribe routine equipment within your scope
  • Maintain accurate written and computer-held records
  • Contribute to service development and provide feedback on patient care
  • Work flexibly to meet service needs, including weekends and bank holidays

Requirements

  • NVQ Level 3 or equivalent in Health/Rehabilitation
  • Care Certificate or willingness to obtain
  • Experience working in a rehabilitation setting
  • Competency in clinical observations and basic diagnostics
  • Good communication skills and the ability to maintain confidentiality

Desirable Skills

  • Experience working in community settings
  • Experience working in a multidisciplinary team
  • Basic IT skills and experience with record-keeping
