Requirements
- At least 2 years’ experience in a community mental health or social care setting
- NVQ level 3 or equivalent, or commitment to obtain within 2 years
- Knowledge of care planning, risk assessment, and mental health legislation
- Strong communication skills and confidence working with service users and professionals
- Basic IT skills including word processing and email
- Compassionate nature with resilience to support people in crisis
Salary Details
This role offers £27,485 to £30,162 per year, with pay progression opportunities and full NHS benefits.
About the Role
As a Senior Community Support Worker, you’ll play a vital part in the Crisis Resolution and Home Treatment Team. Your work will centre on supporting individuals with complex mental health needs, helping them remain in their own homes and communities. From delivering 48-hour follow ups after inpatient discharge to updating care plans, you’ll ensure care is safe, timely, and person centred.
You’ll also provide supervision for Band 3 colleagues, contribute to risk assessments, and work hand in hand with registered practitioners to keep the team running smoothly. Each task matters, whether it’s providing telephone support in moments of distress or liaising with agencies to coordinate holistic care. This is impactful work that keeps people grounded, connected, and hopeful.
Key Responsibilities
- Provide timely follow up visits so service users feel supported after leaving hospital
- Review home treatment plans and update records to keep care responsive
- Discharge service users safely with the right follow up arrangements in place
- Liaise with statutory and voluntary agencies to ensure joined up support
- Deliver clear discharge summaries so information flows smoothly across services
- Supervise Band 3 Community Support Workers to build team confidence and skills
- Offer telephone advice to service users, carers, and other professionals during crises
- Work closely with registered practitioners to maintain effective daily operations
