  • NVQ Level 2 or 3 in health or social care
  • Sound understanding of mild to moderate mental health problems
  • Evidence of ongoing professional development
  • Experience working in a multi-disciplinary team

About the Role

This is a chance to join the Mental Health Access Team in Central Cheshire and play a vital part in supporting people beyond clinical pathways. Instead of relying solely on psychological interventions, you’ll link clients with local community resources that boost their wellbeing and independence. Your role is about listening, understanding, and connecting. By researching and networking with third sector organisations, you’ll guide clients toward services that can truly change their lives. Every conversation and referral you make has the potential to lift someone’s mental health and give them tools to manage better.

Key Responsibilities

  • Build relationships with community organisations to connect clients with tailored services that match their needs
  • Share up to date advice with colleagues about available local resources to improve signposting
  • Research and maintain knowledge of community and third sector options that enhance client wellbeing
  • Work closely with the clinical team to ensure seamless care and meaningful outcomes
  • Provide short term but impactful support that empowers clients to take ownership of their health and wellbeing
