Job Overview
Livewell Southwest is seeking a dedicated Social Worker for their Discharge to Assess 3 Pathway. This role offers an exciting opportunity to work in complex situations, making a real difference in people’s lives. The successful candidate will be part of an integrated health and social care workforce, adhering to the core principles of the social work profession.
Key Responsibilities
- Discharge statutory social care functions on behalf of the organization
- Undertake complex and proportionate assessments, including Mental Capacity and Care Act assessments
- Arrange and review the delivery of social care support
- Support people in taking risks in complex situations to promote their outcomes and wishes
- Conduct safeguarding investigations and consider best interest decisions
- Prepare legal documents for court and represent the organization when required
- Collaborate with colleagues to ensure high-quality, safe, and compassionate care
- Supervise and train students and Band 5 colleagues as needed
Qualifications and Skills
- Qualification in Social Work (postgraduate diploma, bachelor’s degree, or master’s level)
- Registration with the relevant social work regulatory body
- Strong knowledge of social work theories, models, and legal frameworks
- Experience in the health and social care sector
- Excellent communication and interpersonal skills
- Ability to work in an integrated service and multidisciplinary team
