Requirements
- HCPC registration as an Occupational or Physio Therapist
- Degree or diploma in OT or PT from a recognised awarding body
- NHS acute experience with frailty or community pathways
- Confidence fitting both basic and complex equipment
- Great communication skills across multi-disciplinary teams
- Experience with physical dysfunctions and safe discharge planning

Band 6 NHS pay scale
Starting at £39,205 rising to £47,084 depending on experience
About the Role
This is a hands-on opportunity for a passionate therapist who wants to make a difference fast. Based in A&E and acute assessment areas, this role’s all about acting quickly and smartly to support safe, effective discharges. You’ll be a key player in an Integrated Discharge Team, working shoulder-to-shoulder with nurses, social care practitioners, and geriatricians.
Every patient matters here. You’ll be assessing older adults in crisis, often navigating a tangle of physical, medical, and social needs. It’s complex, but rewarding. You’ll not only drive clinical outcomes, you’ll shape how the service grows. Whether you’re already at Band 6 or a Band 5 ready to step up, this is your shot at meaningful work and career growth.
Key Responsibilities
- Deliver specialist OT or PT assessments in emergency and acute settings
- Recommend and fit equipment that supports patient independence
- Work closely with families and carers to enable safe transitions home
- Supervise junior staff and mentor students on clinical placements
- Collaborate with other professionals to ensure timely, holistic care
- Support ongoing service development and audit projects
