Requirements
- Level 3 qualification in a relevant field such as care, health, or rehabilitation
- Completed STARS stroke training or equivalent
- Confident working independently and managing your own caseload
- Great communication and teamwork skills
- Proven experience supporting patients in clinical or community settings
- Sound IT knowledge, including MS Word and Outlook
- Reliable, adaptable, and passionate about impactful work
- Able to manage emotional situations and cope in challenging environments
Salary Details
Salary is based on Agenda for Change Band 4, between 26,928 and 29,551 GBP annually, depending on experience.
About the Role
This role is built around transformation. You’ll help people recently discharged from hospital navigate the early stages of recovery from brain injury or stroke. Whether it’s supporting mobility, guiding communication therapies, or encouraging confidence through daily tasks, every day brings new wins.
You’ll be part of a blended rehab team that includes physios, OTs, psychologists, and dietitians—all working together to get people back to living fuller lives. There’s also room to grow. Through the AGORED Level 4 practice-based qualification, you’ll expand your clinical knowledge and open doors to future progression.
It’s real work, with real heart, and it happens right where recovery starts—at home.
Key Responsibilities
- Deliver rehab programmes created by registered clinicians
- Monitor patient progress and adapt activities based on needs
- Work independently across home and community settings
- Support virtual and face-to-face group sessions
- Build therapeutic relationships with patients and their families
- Keep accurate records and share progress with the wider team
- Contribute to service improvement by sharing ideas and feedback
