Requirements

  • NVQ level 4 in a relevant area OR NVQ level 3 plus diploma training or equivalent experience
  • Strong communication and interpersonal skills to build trust and rapport

Salary Details

This Band 4 role offers a salary of £27,485 to £30,162 per year, pro rata, under the Agenda for Change pay scheme.

About the Role

This role is part of the Support, Time and Reablement (STARS) team, where you’ll be directly supporting adults in the community who are receiving secondary mental health services. Your mission is to help people maximise independence, rebuild confidence and reconnect with meaningful routines and networks.

You’ll spend your time providing practical support, guiding people towards resources like benefits or community groups, and helping them nurture social networks that truly matter. By monitoring wellbeing and spotting early signs of distress, you’ll play a crucial role in preventing relapse and keeping recovery on track.

It’s a role that balances compassion with action. Every visit, every conversation and every piece of support you offer helps someone move closer to living the life they define as “a good life.”

Key Responsibilities

  • Help people regain independence by building daily living skills that boost confidence
  • Encourage meaningful community engagement to reduce isolation and strengthen resilience
  • Support access to practical resources such as benefits, voluntary opportunities and social networks
  • Work closely with families, carers and professionals to deliver joined-up care
  • Spot early signs of relapse and act promptly to safeguard wellbeing
  • Contribute actively to planning and review meetings, ensuring care stays person centred
