Job Description:
The Leeds and York Partnership NHS Foundation Trust (LYPFT) is seeking compassionate and experienced Support Workers to join their team. This role involves supporting individuals with learning disabilities in a supported living environment, promoting independence and helping them achieve their personal goals. The ideal candidate will have experience with Positive Behavior Support (PBS) and postural management.
Key Responsibilities:
- Assist with daily living activities, including personal care, meal preparation, and household tasks.
- Support individuals in accessing community activities and services.
- Follow support plans and promote independence.
- Provide healthcare interventions and follow positive behavior support plans.
- Maintain accurate records and communicate effectively with team members, families, and other professionals.
- Encourage social interaction and help individuals build relationships.
- Offer emotional support and companionship.
Requirements:
- Previous experience working with people with learning disabilities.
- Experience in a supported living environment.
- Strong communication and interpersonal skills.
- Relevant qualifications in health and social care (e.g., Level 2/3 diploma in care or equivalent).
- Flexibility to work shifts, including early, late, night, weekends, and public holidays.
- A commitment to providing high-quality, person-centered care.
