Requirements

  • Passion for helping others and making a real difference in people’s lives
  • Care Certificate or equivalent skills and experience
  • Basic knowledge of safeguarding and community care regulations
  • Willingness to undertake required training
  • Experience in providing care and support to the elderly or people with learning disabilities (desirable)

About the Role
As a Support Worker at Wiltshire Support at Home, you will be part of a team providing short-term homecare services to those recently discharged from hospitals or individuals requiring rapid response support at home. The role involves preventing, reducing, and delaying the need for long-term care by promoting independence. You will be working closely with people in the community to help them live their best lives. This role requires availability on a rota basis from 7am to 10pm, 365 days a year.

Key Responsibilities

  • Provide short to mid-term homecare support to promote independence
  • Assist individuals recently discharged from hospital or under the care of the Rapid Response Service
  • Support individuals in regaining their independence and preventing long-term care needs
  • Work collaboratively with the team and other healthcare professionals
  • Ensure the delivery of high-quality care services
