Requirements
- Intermediate NVQ or equivalent qualifications
- Strong literacy and numeracy at GCSE level
- Practical knowledge of TB treatment, medications, and adherence challenges
- Experience working with socially excluded individuals
- Ability to assess clients holistically, including mental health and financial wellbeing
- Competent using IT systems like Word, Excel, and clinical tools
- Open-minded, adaptable, and comfortable working under pressure
- Fluent in a second language? That’s a plus, especially if it connects with the patient base

The annual salary ranges between 31,944 and 34,937, in line with Agenda for Change Band 5 pay scale
About the Role
This is not your typical support role. You’ll be out in the community—visiting patients in hostels, homes, clinics—wherever they are. You’ll be the steady link between their medical treatment and the real-life hurdles that threaten to derail it. Think of it as frontline public health, wrapped in empathy and boots-on-the-ground persistence. You’ll collaborate with experienced TB nurses, provide directly observed therapy, and help clients overcome the everyday chaos that puts their health at risk.
This is about building trust. You’ll check in regularly, monitor side effects, help with medication routines, and tackle the social issues that stand in the way. Some clients are rough sleepers. Some battle addiction. All of them need someone consistent, compassionate, and unafraid of complexity.
Key Responsibilities
- Support individual TB patients to stick with and complete treatment
- Apply clinical risk assessments and adjust care based on patient complexity
- Develop holistic care plans addressing both medical and social challenges
- Visit patients at home and in hostels to deliver directly observed therapy (DOT)
- Plan daily routines for team outreach and patient visits
- Help patients access wider health services through the mobile x-ray unit
- Organise and support health promotion activities for the wider community
- Partner with nurses, caseworkers, and external services to boost treatment adherence
