Requirements

  • Health and Social Care NVQ Level 3 or higher
  • Confident in high-pressure environments with challenging service users
  • Solid experience in secure, forensic, or prison settings
  • Skilled in conducting assessments and building care plans
  • Ability to manage a caseload with empathy and professionalism
  • Understanding of the CPA process and offender healthcare frameworks
  • Comfortable working both independently and within a multi-disciplinary team
  • Great communicator, ready to liaise with agencies, families, and communities

£33,094 to £36,195, including Inner London HCAS

About the Role

You’ll be that critical bridge between prison and community, helping people rebuild their lives from day one of release. Working within HMP Wormwood Scrubs, you’ll support mental health service users from inside the prison to their first 24 hours post-release and beyond. Your day might start with a prison-based assessment and end with walking someone to their first community support meeting.

This role is about trust, consistency, and action. You’ll track individuals through their sentence, make release plans that actually stick, and ensure the right care and support are waiting for them outside. Working with the Transfer and Release Team, you’ll have a direct hand in creating holistic, forward-thinking plans that give people a real shot at staying well.

Key Responsibilities

  • Assess individuals pre-release and help co-create practical, person-centred plans
  • Meet clients on release day and offer in-person support during their first 24 hours
  • Link service users with healthcare, housing, and mental health services in their release area
  • Develop strong partnerships with national support agencies
  • Facilitate risk assessments and build realistic management plans
  • Actively contribute to the CPA process and documentation
  • Manage a small caseload with professionalism, discretion, and empathy
  • Support group sessions with multi-disciplinary staff when appropriate
