Requirements

  • Registered Nurse (Child) or Registered Midwife with active NMC registration.
  • Minimum 5 years post-registration experience in neonatal or transitional care.
  • In-depth understanding of neonatal care standards and national guidance (BAPM, NICE).
  • Strong leadership and team coordination abilities.
  • Good knowledge of safeguarding policies and procedures.
  • Confident communicator with families and multidisciplinary teams.
  • Organised, independent, and skilled at managing priorities.
  • Practice Assessor or Supervisor qualification (desirable).
  • Proven record in service improvement or innovation (desirable).

Salary Details

This is a Band 7 role under the Agenda for Change pay scheme, offering £47,810 a year per annum, pro rata for part-time work.

About the Role

As a Transitional Care Lead Nurse, you’ll guide the development and delivery of care for newborns who need extra support after birth but don’t require admission to a neonatal unit. These might be babies needing help with temperature regulation, feeding, jaundice treatment, or those affected by maternal substance use.

You’ll work with maternity, neonatal, and community teams to ensure families receive seamless, family-centred care. It’s a role that balances hands-on clinical expertise with leadership and innovation. You’ll also help shape the future of care through quality improvement, mentoring, and service development initiatives, including the exciting potential creation of a Neonatal Outreach Team.

Key Responsibilities

  • Lead the coordination and quality of the transitional care service to keep mothers and babies together.
  • Develop and monitor individualised care plans tailored to each baby’s needs.
  • Support safe, early discharge by collaborating with multidisciplinary teams.
  • Promote family-integrated care to strengthen parent–infant bonding.
  • Mentor and educate nursing and midwifery staff in best practices.
  • Drive continuous improvement through audits and service development projects.
  • Oversee infection control, documentation, and safeguarding standards.
  • Help plan and establish the Neonatal Outreach Team to support families post-discharge.
