Job Summary
Kent Community Health NHS Foundation Trust is seeking an enthusiastic and adaptable Urgent Care Service Assessor to join its progressive team in Folkestone. The successful candidate will work in a community-based setting, supporting patients with complex needs and ensuring services are patient-focused.
The role involves working closely with primary care colleagues to deliver targeted, high-quality care while fostering a sense of community within the team.
Main Duties & Responsibilities
- Assess, plan, and review care needs for patients with complex conditions.
- Collaborate with a multidisciplinary team to provide high-quality, safe care.
- Work proactively in a short-term service to enhance patient experience.
- Drive between patient homes and work sites (own transport required).
- Develop assessment and treatment skills with the support of senior staff.
Person Specification
Qualifications
Essential:
- Foundation Degree Level study (Level 5) or equivalent (Nursing Associate) OR willingness to work towards this.
- Functional Skills at Level 2 (Literacy and Numeracy) or GCSE Grade A-C (or equivalent) in English and Maths.
Desirable:
- Mentorship/Coaching qualification.
Experience
Essential:
- Minimum of 2 years’ experience as a Senior Healthcare Worker.
- Experience working within a team environment.
- Proficiency in Microsoft Office and IT systems (e.g., E-Pay, E-Roster, AT-P, Rio).
Desirable:
- Experience in a community healthcare environment.
Knowledge
Essential:
- Understanding of NHS policies, Clinical Governance, NICE guidelines, Essence of Care, and National Urgent Care Agenda.
- Awareness of up-to-date clinical practices.
- Understanding of the role of a Band 4 Assessor.
Skills & Abilities
Essential:
- Strong communication and interpersonal skills.
- Ability to work independently and use initiative.
- Ability to adapt to change.
- Moderate IT proficiency.
- UK driving licence & access to a vehicle.
